Filesystem activity monitoring

Hello, misc.

I wonder, is there some software that could show me in top(1) manner 
which files are accessed most intensively right now and by which 
process/user? The main question I wish to answer is why my OpenBSD/4.2 
router's hdd lights are blinking while it definitely should be idle (or 
not? that is the question).

fstat(1) gives just a snapshot and it's ..err... not much friendly in 
that sense :)
